THE APPLICATION OF ORTHOPNEIC POSITION IN MEETING OXYGENATION NEEDS IN CKD PATIENTS AT RSUP DR. SARDJITO YOGYAKATRA

Abstract

Background: CKD is a noncommunicable disease with a global prevalence of 14.2% in 2023 and a prevalence of 6.1% in DIY. One of the most common complications I dyspnea caused by fluid overload and cardiopulmonary dysfunction. Orthopneic positioning, as a non pharmacological intervention, can optimize lung expansion and improve oxygen saturation in patients with CKD.

Objective: Gained practical experience in applying the orthopneic position to meet the oxygenation needs of CKD patients at Dr.Sardjito General Hospital in Yogyakarta.

Methods: This study employed a descriptive design using a case study approach involving two patients with chronic kidney disease (CKD) who experienced dsypnea. Nursing care was provided through assessment, diagnosis, intervention, implementation, and evaluation, utilizing and evidence-based clinical responses of the patients.

Results: The use of the orthopneic position for three days in both patients resulted in progressive imporement in respiratory function. Mr. A’s respiratory rate decreased from 27 to 20 breaths per minute and his SpO₂ increased from 95% to 98%, while Mr. B’s respiratory rate decreased from 27 to 21 breaths per minute and his SpO₂ increased from 95% to 98%. Shortness of breath decreased and nursing issues were resolved in both patients.

Conclusion: The use of the orthopneic position 3 days (24 hours each day) for 20 minutes per session was effective in reducing oxygenation problems in patients with CKD.
